Please fill it with your data, save the file, print it, sign and send the scanned form directly to the Hotel (email@example.com). We will check the participants’ list together with the representatives of Hotel.
According to our agreement we can book not only 3 nights for Conference, but 2 ones before and 2 after, it means you can book room from 24 to 30 September 7 nights, you can arrive on Sunday before and leave Sunday after the Conference.
